IT IS exciting to read about the current efforts hospitals and others are making toward establishing "medical homes," as described in "A more welcoming model for care" (Health/Science, May 19). But it might surprise you to know that Urban Medical, in Jamaica Plain, is a pioneer in this model of primary care upon which these newer efforts are designed. For 30 years, we have been our patients' "medical home," using a team model, and caring for patients wherever they are - in nursing homes, assisted-living facilities, a doctor's office, or yes, even at home.
We at Urban Medical look forward to working with Dr. Allan Goroll and others as we try to spread the model to patients across the city and beyond.
