Woman’s death in Andover prompts inflatable pool slide recall Date: May 10, 2012 Units: About 21,000 Walmart and Toys R Us recalled Banzai inflatable pool slides after a 29-year-old Colorado mother died in Andover. She fractured her neck going down the in-ground pool water slide and hit her head at the bottom of a partially deflated slide. Two other serious injuries were reported, including another fractured neck and leg paralysis.
