6. Send your report to all credit agencies and companies involved

Send a copy of your identity theft report to the fraud departments of the three major credit reporting agencies, including a cover letter addressed to each company and any supporting documents, advises the FTC.

You will also need to send a report to each company that lists where the thief used your personal information, and any other helpful information, such as whether existing accounts were involved or if the thief opened new ones.
