Alright, it's more funny than scary, but it ranks at the top of my list of favorite novelty songs. Along with Weird Al's "Eat it" and Julie Brown's "Homecoming Queen's got a Gun".
The more serious choice would be "Black Sabbath" by Black Sabbath from the LP Black Sabbath.

On topic - I loved "They're Coming To Take Me Away" as a kid - heard it all the time on the Dr. Demento radio show (along with classics like "Don't Eat The Yellow Snow"). -

Any list of scary songs that does not include "The Turkish Song of the Damned" by The Pogues is incomplete... You could include several Pogues songs, but I will throw that one out there because it is probably their scariest... Between Shane MacGowan's shrieking, the menacing lyrics and the haunting music, this song easily takes my vote! -

How about "Holiday in Cambodia" by the Dead Kennedys, or "Rooster" by Alice In Chains. What about anything by Black Flag. "Hurt" from Nine Inch Nails is also pretty disturbing. -

There are different kinds of scary. Alice Cooper built a career around horror schlock like 'Welcome to My Nightmare'. Black Sabbath made a frightful amount of money invoking Satan. Blue Oyster Cult had songs about Godzilla, Nosferatu and a supernatural being called 'Imaginos' who manipulated human events.
But none of that material is terribly scary in the sense of actually giving you chills down the spine when you listen it, because it's removed from reality. Some songs that I have found to be truly creepy are 'Master of Sparks' by ZZ Top, 'Brand New Day' by 10CC and 'I Hung My Head' by Sting. But you really have to listen closely to the lyrics. -

The debut album by Ernie Boch Junior and the Hired Mechanics occupies my entire top 10 of scary songs. Oddly enough this same record is all over my top 10 comedy sketches too. They're very versatile. -

Jim Carrol - All People Who Died (not sure if that is the exact title)
Not really scary, but so damn honest. Having spent most of my teen years (70's) in the East Village of Manhattan, you saw this every day.
When I was young and stoned, Revolution #9 and Iron Man scared the hell out of me. -

It's clearly tongue-in-cheek, but when Tonio K goes frothing mad at the end of H-A-T-R-E-D, it's not difficult to imagine him actually killing his ex-girlfriend. That's kind of scary. -

Revolution #9 is a beauty. And I can confirm that when you play the 'number nine' part backwards it really does say 'turn me on dead man'. John Lennon always denied that the 'Paul is dead' stuff was deliberate but it obviously was. Why else would he say 'Here's another clue for you all, the walrus was Paul' on Glass Onion? -

Yes it's not much of a piece of music. John and Yoko were on some very serious drugs there. I imagine George Martin was mortified that it was included on an album. -
