Failures cited at West Roxbury fire

A report from the National Institute for Occupational Safety and Health cited several missteps in efforts to fight a 2007 fire in a West Roxbury restaurant that killed two firefighters. The fire had been burning and fumes building up for an hour in an enclosed space above the ceiling before smoke was discovered and firefighters were called.
