The state’s major health insurers plan to raise premiums by about 10 percent next year, prompting many employers to reduce benefits and shift additional costs to workers.

The Food and Drug Administration approved the new swine flu vaccine yesterday, a long-anticipated step as the government works to get vaccinations underway next month.

Pfizer Inc.’s Pharmacia & Upjohn unit pleaded guilty to a felony violation of the Food, Drug and Cosmetic Act for misbranding the drug, Bextra, with the intent to defraud or mislead.
