A pharmacist handed over his license to state officials yesterday, a day after police took him into custody in Sandwich, where authorities said he was intoxicated while at work.
Frederick Beal, 51, of Lakeville, voluntarily gave up his license, said Jean Pontikas, director of the state's licensing division. ''He will no longer be able to practice in Massachusetts," she said.
Beal did not return phone calls to his home yesterday afternoon.
He had been employed at Sedell's Pharmacy in Sandwich since June and was licensed in Massachusetts since last year. He also holds licenses in New Hampshire and Maryland.
Beal's New Hampshire license was revoked in 1984 after a State Police investigation, prompted by his supervisor's discovery of missing codeine medications at a pharmacy in Cheshire County, found that he forged prescriptions for personal use, said Paul Boisseau, executive secretary for the New Hampshire Board of Pharmacy. Beal was charged with a misdemeanor and served two years probation.
