PROVIDENCE, R.I.—State officials have said an application for a Boston-based hospital chain to merge with a financially troubled medical center in Woonsocket is incomplete.

Steward Health Care Systems and Landmark Medical Center have been given a two-week extension to submit the remaining documents needed to complete their application. The application will be reviewed by the state Department of Health and the Attorney General's office.

Health Department Director Michael Fine called it a "one-time extension." The requested information is due Jan. 11.

A state judge in June approved plans for Steward to acquire Landmark, and the two filed their formal application to the state in October.
