Correction: Because of an editing error, a Page One headline yesterday on a story about a fourth patient possibly contracting the rare brain disease progressive multifocal leukoencephalopathy after being treated with Bigoen Idec Inc.'s multiple sclerosis drug Tysabri incorrectly referred to the case as ''a fourth death." The patient is still alive. In the three earlier, confirmed cases, two patients died and one is alive.
Correction: Because of a reporting error, the Radio Tracks column in yesterday's Living/Arts section misspelled the name of Lake Wobegon in an article about Garrison Keillor and ''A Prairie Home Companion."
Correction: Because of an editing error, a story in yesterday's Globe West section gave the incorrect date for the dedication ceremony for Salvatore A. Balsamo Millennium Park next to Newton City Hall. The event is Monday at 5:30 p.m., with a rain date of Tuesday at 5:30 p.m.
