Visiting Nurses receive grant to expand Telehealth Program

Arlington-based Visiting Nurse and Community Health, Inc. has been awarded a grant to expand its Telehealth Program to monitor medically fragile patients and decrease emergency room visits and hospitalizations.

The Symmes Medical Use Nonprofit Corporation awarded the $23,500 grant to the non-profit Visiting Nurse and Community Health for the Telehealth Program, which monitors home healthcare for the agency’s congestive heart failure and chronic obstructive pulmonary disease patients.

Registered nurses working with physicians in the Telehealth Program remotely monitor homebound patients’ blood pressure, heart rate, weight oxygen levels and temperatures. Any fluctuations result in a phone call to the patient to check on his/her status, and a follow-up call to the patient’s primary care nurse or physician, depending on the situation.
