A 21-year-old Newton man has been cited with negligent motor vehicle homicide and other charges in connection with a head-on crash last summer that killed a 43-year-old Chelsea man, prosecutors said Thursday, Jan. 6.

Evan Hoffman was cited with negligent motor vehicle homicide, speeding, marked lanes violation, and driving without insurance in the crash, which resulted in the death of Jose Puzul-Perez, the Middlesex district attorney’s office said in a release.

No arraignment date has been set.

At about 6:12 a.m. on June 26, Newton police responded to the collision near 428 Beacon St. Upon arrival, officers found the victim’s vehicle, a 1994 Ford Aero Star, resting in the westbound lane of traffic.

The victim was trapped inside the vehicle and was unresponsive. Newton fire officials extracted him from the vehicle, and he was transported to Newton-Wellesley Hospital, where he was pronounced dead.

The second vehicle involved in the crash was a 2006 Jeep Grand Cherokee allegedly driven by Hoffman, a Waban resident who refused to receive medical treatment at the scene, prosecutors said.

Investigators from Newton and State Police concluded that Hoffman was allegedly driving west on Beacon Street at a high rate of speed when his vehicle crossed over the double yellow center line and moved into the on-coming lane of traffic, the DA’s office said.

Hoffman’s vehicle struck the victim’s van, which was traveling east on Beacon Street, prosecutors said.
