The family of a woman killed by a flying manhole cover on the Southeast Expressway in 2016 has filed a wrongful death lawsuit against the state Department of Transportation and two private contractors.

Relatives of Caitlin M. Clavette filed their suit in Suffolk Superior Court on Monday.

The 6-page complaint alleges MassDOT and the contractors failed to prevent “an obviously mis-matched and severely deteriorated 200-pound manhole cover’’ from endangering the public and ultimately smashing through Clavette’s windshield, killing her.
